Tillamook County Wellness (TCW) has launched a community health needs survey to identify and prioritize the most pressing health issues for local residents. Since its establishment in 2016, TCW, a program of Tillamook County Community Health Centers, has collaborated with over 40 organizations, including Adventist Health Tillamook, Nehalem Bay Health Center, and Tillamook Family Counseling Center to enhance community health.
As part of the required Community Health Needs Assessment (CHNA) cycle, which is updated every three years, this survey is a crucial tool for gathering local input. Your feedback will help shape the Community Health Improvement Plan (CHIP) for the next cycle.
Throughout 2024, TCW partners are leading primary data collection with interviews, focus groups and this short, anonymous survey that gives local residents the opportunity to weigh in on the quality of life in our county.

This survey allows us to hear directly from our friends and neighbors about their top health needs,” says Eric Swanson, President of Adventist Health Tillamook, the organization coordinating the CHNA report. “We are dedicated to collaborating with our community to build a healthy and happy future for everyone who calls Tillamook County home.”
