Tillamook Health & Human Services wants to know how prepared you are for a natural disaster, and how they can better support you when emergency events happen. Your feedback will help improve emergency services and supports in Tillamook County.
To take the survey, you must be:
- Living in Tillamook County, Oregon
- At least 18 years old
Survey participants can enter in a drawing for one of three $100 VISA gift cards.
Your survey responses will be anonymous and confidential, however, a valid mailing address is required to be entered into the drawing.
